State-wide Equipment Program (SWEP)
Aidacare is a contracted supplier for Mobility and Rehabilitation Equipment as well as Home Modifications for eligible State-wide Equipment Program (SWEP) clients and National Disability Insurance Scheme (NDIS) participants in Victoria.
Aidacare’s equipment consultants are available to demonstrate and trial items at any location, including client’s private homes. We are highly experienced and qualified provider of home modification services assisting people to continue to live safely and independently in their own home. Home modifications are based on a thorough assessment by an Occupational Therapist and can range from something as simple as installing grab-rails, hand-rails, lever taps and step-wedges to more complex home modifications such as installing fixed wheelchair ramps, widening doorways and modifying bathrooms.
Our installers work collaboratively with healthcare professionals, clients and carers to determine the needs of all parties involved, incorporating the choices of the SWEP/NDIS Clients into the plan for Home Modifications and clear communication is maintained throughout the process.
What is SWEP?
A. SWEP provides a subsidy towards the cost of equipment and/or modifications to eligible participants. This is to improve independence in your home, assist in community participation and support families and carers in their role.
Am I eligible?
A. All registered SWEP participants should have a Web Application Number (WAN). If you are a new client to SWEP applying for a Victorian Aids & Equipment Program (VAEP) or Community Equipment Program (CEP) program you may need to complete the Eligibility Form with all your personal details. You will need to make an appointment to see your treating doctor/specialist to complete the last page of the Eligibility Form on your behalf. This is to confirm that you are eligible to receive assistance from SWEP.
How do I order?
A. An order for aids, equipment or home modifications will be placed on your behalf by a allied health professional who will submit the request to SWEP for assessment. Once the order has been approved and Aidacare receive the approval paperwork from SWEP we will contact you to arrange delivery and/or installation.
How do I arrange a Home Modification?
A. Modifications are based on an in-home assessment by a therapist and can range from something as simple as installing grab-rails, hand-rails, lever taps and step-wedges to more complex home modifications such as installing fixed wheelchair ramps and widening doorways.
