The Australian Government’s new Support at Home Program has replaced the existing Home Care Packages and Short-Term Restorative Care programs effective November 1st. This reform introduces a simplified model where a single registered provider manages each participant’s care, including the delivery of Assistive Technology (AT) and Home Modifications (HM). Data Collection Process
The AT-HM Data Collection process is open until February 2026, allowing providers to request extra funding for clients transitioning to Support at Home. This applies where unspent HCP funds are not enough to cover a participant’s required assistive technology or home modifications.
AT-HM Under Support at Home – Pre-Approval for AT Over $15,000
A new Pre-Approval Form is required for any assistive technology over $15,000 under Support at Home. Providers simply upload the form with prescriptions and quotes to My Aged Care, validate the submission, and await approval for the participant’s AT High Tier funding to be increased so the item can be purchased.

What is my role as a prescribing therapist under the Support at Home AT-HM scheme?
A. Prescribing therapists are responsible for assessing functional need, prescribing appropriate AT/HM items, ensuring item suitability, providing clinical justification, and delivering any required wraparound services such as training, setup, and follow-up. Prescription is mandatory for any “prescribed” items on the AT-HM List.

How do I know which items require a prescription?
A. The AT-HM List classifies every item as low risk, under advice, or prescribed. Prescribed items always require a suitably qualified health professional to complete a prescription. Items listed as “under advice” require professional guidance but not a formal prescription. Aidacare have identified this for you on the product detail pages – look for (i) Support @ Home.

Do I need to consider a participant’s funding tier when prescribing?
A. Therapists prescribe based on clinical need, not the funding tier. However, therapists should understand the tier limits (low $500, medium $2000, high $15,000+) so they can help the provider plan procurement, quote gathering, and whether additional evidence is required for high-tier items.

How do I support participants transitioning from Home Care Packages?
A. Transitioned HCP participants automatically gain approval for AT-HM. Therapists may be asked to assist in identifying needs, producing prescriptions for items previously planned under HCP, and supporting the provider with evidence for data-collection funding requests (October 2025 to February 2026).

What documentation is required to support a prescription?
A. Documentation typically includes a clinical assessment summary, justification for the item, details of alternatives considered, specifications or configuration requirements, and any quotes if requested by the provider. This evidence must align with the AT-HM List.

What are wraparound services and when do I need to provide them?
A. Wraparound services include training, setup, fitting, adjustment, environmental checks, and follow-up to confirm the AT/HM is effective and safe. These services are fully government-funded under clinical care, with no participant contribution.

What happens if the participant’s need changes after equipment is prescribed?
A. The provider can request a Support Plan Review or reassessment. Therapists may be required to reassess and re-prescribe items if needs change significantly, or if repairs/replacements are needed for previously funded items.

Do therapists prescribe home modifications differently from AT?
A. Home modifications always require a qualified health professional (typically an OT) to prescribe. The therapist must describe functional need, specify the modification required, ensure it is compliant with building standards, and work with builders/suppliers to verify correct installation.

Can I prescribe items not listed on the AT-HM List?
A. No. AT-HM funding can only be used for items on the AT-HM List. If an item is not listed, it cannot be funded through the AT-HM scheme and alternative funding pathways should be explored.

How do AT-HM responsibilities interact with ongoing Support at Home services?
A. The prescribing therapist focuses only on prescription and clinical wraparound services. The Support at Home provider manages procurement, coordination, installation, payment, and claiming through Services Australia. Therapists do not manage funds.
